CnoteMD is just a slight modification of a nickname I've been using since the early 90's when I was using IRC (long before the yahoo messengers and other forms of online chat rooms).

I started using the nickname c-note (slang for $100 bill). People always thought that I was good with money, though at the time I had none. A few years later, I finished med school and added the MD.
