Any obvious errors aren't on chef's as chef's don't bring plates of food out to customers, servers do.
If the manager brings out the food and the order was put in correctly from the server that took the order, then the manager is at fault for
SERVING IT WRONG!
The hostess doesn't bring you food, so they have nothing to do with food.
